Parkdale Queen West Community Health Centre is seeking an Interim Nurse Practitioner for the HART Hub to provide holistic primary care during parental leave coverage at the Queen West site in Toronto. The NP will work with multidisciplinary teams across substance use care, mental health and health promotion to ensure continuous care and program development.

Qualifications

  • Current registration as a Registered Nurse (Extended Class) with the College of Nurses of Ontario; RNAO membership.
  • 3 to 5 years nursing experience in community, public health and/or primary care settings.
  • Experience with street or outreach nursing is an asset.
  • Knowledge of mental health, substance use care, harm reduction and chronic disease.
  • Strong oral and written communication skills.
  • Strong problem solving and adaptive skills.
  • Excellent decision-making skills.
  • Understanding of anti-oppression principles and frameworks.
  • Comfort with diverse populations; experience with homeless populations and people who use drugs.
  • Able to work independently and as part of a multidisciplinary team.
  • Second language is an asset.

Responsibilities

  • Performs clinical tasks including history taking, routine diagnostics, diagnoses and care plans, and addictions treatment.
  • Provides brief counselling and support for mental health and substance use needs; makes referrals as appropriate.
  • Identifies trends in health status to inform planning and advocacy.
  • Establishes clinics at partnering agencies in collaboration with clinical and non-clinical staff.
  • Performs routine outreach with internal Centre programs/leads as appropriate.
  • Provides primary care during the treatment period and coordinates wraparound services.
  • Maintains timely documentation in PQWCHC’s EMR to meet funder and regulatory requirements.
  • Leads development of substance use care protocols as required.
  • Participates in research, evaluation, and knowledge-sharing activities.
