Job summary

CarePartners is seeking a Nurse Manager to lead and mentor nursing teams across Niagara Falls, Grimsby, Beamsville, St. Catharines and surrounding areas. Based in the St. Catharines office, you will coordinate care plans and ensure high-quality patient care in the community.

The role offers a competitive salary ($79,170.00–$99,528.00) with benefits and flexible work-from-home options. Evening/weekend on-call rotas and professional growth opportunities are included.

Qualifications

• RN degree with active registration with the College of Nurses of Ontario (CNO).
• 3–5 years of relevant leadership/management experience in community health care.
• Excellent verbal and written English communication; French is an asset.

Responsibilities

• Coordinate, supervise, and coach nurses in the field.
• Develop and maintain care plans for Community Health Services.
• Stay current with Regulated Health Professions Act policies and monitor performance.
• Promote Community Health Services and maintain positive public relations.

We are currently looking for someone to join our dynamic Leadership Team in the role ofNurse Manager.

This is a Full-Time position. Hours of work will be Monday-Friday as well as participation in the Branch Manager On-Call, which includes evenings and weekends on a rotational basis.

This position would be working out in the community thoughout the Niagara Falls, Grimsby, Beamsville, St. Catharine's and surrounding areas.

This position would be working out of St. Catharine's office with in office expectations.

• This position offers a competitive compensation grid, with salary progression based on hours worked and role-specific criteria. The salary range is $79,170.00 to $99,528.00, and placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.

• Provides coordination, supervision, and coaching to the Nurses in the field.
• Develops and maintains an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
• Keeps abreast of policies outlined in the Regulated Health Professions Act and Community Health Services best practices and monitoring employee performance.
• Promotes and markets Community Health Services and maintains positive public relations.
• Provides active guidance for workplace Health and Safety.
• Performs other duties as required.

• Registered Nurse (RN) degree
• 3 - 5 years of job-related experience or an equivalent combination of education and experience
• Registration with the College of Nurses of Ontario (CNO)
• Proven computer skills, including email, word processing, work scheduling systems
• Thorough knowledge of case management skills and nursing processes and practices
• Vehicle is required and some regular travel is required
• Leadership and people management skills are an asset
• Excellent English verbal and written communication skills
• French language skills are an asset
• Clear Background and Vulnerable Sector Check
