Job summary

CarePartners is seeking a Personal Support Manager to coordinate and supervise the delivery of Community Health Services. The role is temporary full-time with Monday–Friday hours and on-call rotations including evenings and weekends.

Responsibilities include leading Community Support Workers, developing care plans, and ensuring compliance with health regulations. Travel to service areas is required as needed.

Qualifications

  • RN or RPN degree or diploma required or registered with CNO.
  • 3–5 years of related experience or equivalent.
  • Registration with the College of Nurses of Ontario (CNO) is required.
  • Experience in community healthcare is an asset.
  • Leadership experience is an asset.
  • Proven computer skills including email, word processing, and scheduling systems.
  • Valid driver’s license and regular travel may be required.
  • Strong English communication skills; French is an asset.
  • CPR certified; background and vulnerable sector checks required.

Responsibilities

  • Provide coordination, supervision, and coaching to Community Support Workers.
  • Develop and maintain Care Plans for Community Health Services.
  • Stay informed on Regulated Health Professions Act policies and best practices.
  • Promote and market Community Health Services; maintain positive public relations.
  • Provide guidance for workplace Health and Safety.
  • Perform other duties as required.

Overview

We are currently looking for someone to join our dynamic Leadership Team in the role of Personal Support Manager. The primary role of the Personal Support Manager is to provide effective coordination and supervision of the delivery of Community Health Services.

This is a Temporary Full-Time position. Hours of work will be Monday-Friday as well as participation in the Branch Manager On-Call, which includes evenings and weekends on a rotational basis.

  • This position offers a competitive compensation grid, with salary progression based on hours worked and role-specific criteria. The salary range is $64,935.00 to $82,875.00, and placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.
What The Role Involves

Our leadership team plays a critical role in providing high quality patient care, mentoring our health care teams and inspiring others.

  • Provides coordination, supervision, and coaching to the Community Support Workers in the field.
  • Develops and maintains an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
  • Keeps abreast of policies outlined in the Regulated Health Professions Act and Community Health Services best practices and monitoring employee performance.
  • Promotes and markets Community Health Services and maintains positive public relations.
  • Provides active guidance for workplace Health and Safety.
  • Performs other duties as required.
What You Bring
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • 3 - 5 years of job-related experience or an equivalent combination of education and experience
  • Registration with the College of Nurses of Ontario (CNO)
  • Experience in a community healthcare environment considered an asset
  • Experience in a formal leadership role considered an asset
  • Proven computer skills, including email, word processing, work scheduling systems
  • Thorough knowledge of case management skills and nursing processes and practices
  • Vehicle is required and some regular travel is required
  • Leadership and people management skills are an asset
  • Excellent English verbal and written communication skills
  • French language skills are an asset
  • Current CPR Certification
  • Clear Background and Vulnerable Sector Check
