Job summary

CarePartners is seeking a Personal Support Manager to coordinate and supervise our Community Health Services delivery. The role involves leadership of Community Support Workers and development of care plans in collaboration with Ontario Health atHome.

The position is full-time with Monday–Friday hours and on-call rotation including evenings and weekends. A strong nursing background and leadership experience are required, with travel as needed.

Qualifications

• Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
• 3-5 years of job-related experience or an equivalent combination of education and experience
• Registration with the College of Nurses of Ontario (CNO)
• Experience in a community healthcare environment is an asset
• Leadership experience is an asset
• Proven computer skills including email, word processing, and scheduling systems
• Thorough knowledge of case management skills and nursing processes
• Vehicle is required and some regular travel is required
• Leadership and people management skills are an asset
• Excellent English verbal and written communication skills
• French language skills are an asset
• Current CPR Certification
• Clear Background and Vulnerable Sector Check

Responsibilities

• Provides coordination, supervision, and coaching to the Community Support Workers in the field.
• Develops and maintains an effective Care Plan with respect to Community Health Services.
• Keeps abreast of policies outlined in the Regulated Health Professions Act and CHS best practices.
• Promotes and markets Community Health Services and maintains positive public relations.
• Provides active guidance for workplace Health and Safety.
• Performs other duties as required.

Overview

We are currently looking for someone to join our dynamic Leadership Team in the role of Personal Support Manager. The primary role of the Personal Support Manager is to provide effective coordination and supervision of the delivery of Community Health Services.

This is a Full-Time position. Hours of work will be Monday-Friday as well as participation in the Branch Manager On-Call, which includes evenings and weekends on a rotational basis.

• This position offers a competitive compensation grid, with salary progression based on hours worked and role-specific criteria. The salary range is $64,935.00 to $82,875.00, and placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.

What You Bring
