Job summary

CarePartners is seeking a Personal Support Manager for the Quinte area to coordinate and supervise delivery of Community Health Services. This full-time role includes evening/weekend on-call rotation on a rotating basis.

You will lead Community Support Workers, develop care plans, ensure compliance with health regulations, and advocate for patient care quality. A nursing license and leadership experience are required, with travel to various locations in Ontario.

Qualifications

• Registered Nurse (RN) or Registered Practical Nurse (RPN) with current license.
• 3–5 years of job-related experience or equivalent.
• Registration with the College of Nurses of Ontario (CNO) is required.
• Experience in community healthcare and formal leadership is an asset.

Responsibilities

• Coordinate, supervise, and coach Community Support Workers in the field.
• Develop and maintain an effective Care Plan with respect to Community Health Services, in partnership with Ontario Health atHome.
• Keep abreast of RHPA guidelines and best practices, and monitor employee performance.
• Promote and market Community Health Services and maintain positive public relations.
• Provide active guidance for workplace Health and Safety.
• Perform other duties as required.

Overview

We are currently looking for someone to join our dynamic Leadership Team in the role of Personal Support Manager for the Quinte area. The primary role of the Personal Support Manager is to provide effective coordination and supervision of the delivery of Community Health Services.

This is a Full-Time position. Hours of work will be Monday-Friday as well as participation in the Branch Manager On-Call, which includes evenings and weekends on a rotational basis.

• This position offers a competitive compensation grid, with salary progression based on hours worked and role-specific criteria. The salary range is $64,935.00 to $82,875.00, and placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.

• Provides coordination, supervision, and coaching to the Community Support Workers in the field.
• Develops and maintains an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
• Keeps abreast of policies outlined in the Regulated Health Professions Act and Community Health Services best practices and monitoring employee performance.
• Promotes and markets Community Health Services and maintains positive public relations.
• Provides active guidance for workplace Health and Safety.
• Performs other duties as required.

What You Bring

• Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
• 3 - 5 years of job-related experience or an equivalent combination of education and experience
• Registration with the College of Nurses of Ontario (CNO)
• Experience in a community healthcare environment considered an asset
• Experience in a formal leadership role considered an asset
• Proven computer skills, including email, word processing, work scheduling systems
• Thorough knowledge of case management skills and nursing processes and practices
• Vehicle is required and some regular travel is required
• Leadership and people management skills are an asset
• Excellent English verbal and written communication skills
• French language skills are an asset
• Current CPR Certification
• Clear Background and Vulnerable Sector Check
