Job summary

CarePartners, an Ontario-based home health care provider, is seeking a Nurse Manager to lead Community Nursing Health Services across Niagara Falls, Grimsby, Beamsville, St. Catharines and nearby areas. The role is full-time with in-office expectations at the St. Catharines location and field supervision.

You will coordinate care plans, supervise nursing teams, ensure regulatory compliance and promote high-quality patient care with flexible work-from-home options where applicable.

Qualifications

  • Must be a Registered Nurse (RN) with current CNO registration.
  • Minimum 3–5 years of related leadership/management experience.
  • Thorough knowledge of case management and nursing processes.

Responsibilities

  • Coordinate, supervise, and coach nurses in the field.
  • Develop and maintain Care Plans for Community Health Services, in collaboration with Ontario Health atHome.
  • Monitor policy adherence under Regulated Health Professions Act and best practices; monitor performance.
  • Promote and market Community Health Services; maintain positive public relations.
  • Provide guidance for workplace health and safety; perform other duties as required.

Overview

We are currently looking for someone to join our dynamic Leadership Team in the role of Nurse Manager. The primary role of the Nurse Manager is to provide effective coordination and supervision of the delivery of Community Nursing Health Services.

This is a Full-Time position. Hours of work will be Monday-Friday as well as participation in the Branch Manager On-Call, which includes evenings and weekends on a rotational basis.

This position would be working out in the community thoughout the Niagara Falls, Grimsby, Beamsville, St. Catharine's and surrounding areas.

This position would be working out of St. Catharine's office with in office expectations.

  • This position offers a competitive compensation grid, with salary progression based on hours worked and role-specific criteria. The salary range is $79,170.00 to $99,528.00, and placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.

Our leadership team plays a critical role in providing high quality patient care, mentoring our health care teams and inspiring others.

  • Provides coordination, supervision, and coaching to the Nurses in the field.
  • Develops and maintains an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
  • Keeps abreast of policies outlined in the Regulated Health Professions Act and Community Health Services best practices and monitoring employee performance.
  • Promotes and markets Community Health Services and maintains positive public relations.
  • Provides active guidance for workplace Health and Safety.
  • Performs other duties as required.
What You Bring
  • Registered Nurse (RN) degree
  • 3 - 5 years of job-related experience or an equivalent combination of education and experience
  • Registration with the College of Nurses of Ontario (CNO)
  • Proven computer skills, including email, word processing, work scheduling systems
  • Thorough knowledge of case management skills and nursing processes and practices
  • Vehicle is required and some regular travel is required
  • Leadership and people management skills are an asset
  • Excellent English verbal and written communication skills
  • French language skills are an asset
  • Clear Background and Vulnerable Sector Check
