Job summary

Shawin en Abinoojii seeks a Northern Care Case Manager to provide in-depth case management for families, children, and communities within the Northern Care Program. Thirty-five percent of time is spent in community on rotation.

You will oversee licensing, recruit House Parents, and ensure documentation and program compliance while maintaining strong relationships with First Nation communities.

Qualifications

• Bachelor of Social Work (BSW) or equivalent required.
• Minimum 2 years frontline case management experience.
• Knowledge of community-based resources and how to access them.
• Strong assessment skills and experience conducting investigations.

Responsibilities

• Providing extensive Case Management Services to House Parents and homes in the Northern Care Program.
• Overseeing licensing duties of Northern Care Program homes.
• Recruitment, screening, initial assessments, and interviews of potential House Parents.
• Ensuring proper checks and paperwork for licensing.
• Developing House Parent Care Plans and maintaining detailed case notes.
