Job summary

The Northwest Territories Health and Social Services Authority (NTHSSA) Yellowknife Region is hiring a Nurse Case Manager for a 1-year term. You will coordinate complex care journeys, advocate for patients, and collaborate with physicians, nurses, and other providers within the Integrated Care Team at the Yellowknife Primary Care site.

Responsibilities include managing travel care for high-risk patients, overseeing panel care and preventive screenings, and supporting culturally safe care

Qualifications

• Nursing Degree required and two years of recent nursing experience in ER, Community Health Centre, or Public Health Unit.
• Equivalent combinations of education and experience will be considered.

Responsibilities

• Care coordination for high-risk or complex patients, including travel coordination.
• Panel management for acute and chronic care needs and preventive screenings.
• Advocate for patients and liaise with families and the multidisciplinary team.
