Job summary

Elizz by SE Health in Ontario, ON is seeking an Overnight On-Call Care Coordinator to support after-hours scheduling, client and family communication, and accurate documentation.

You will work within a hybrid schedule, provide on-call support 10:00 p.m. to 7:30 a.m. ET, collaborate with a virtual team, and help maintain care quality across Canada.

The role requires 1+ year of customer service experience, strong communication, and comfort with MS Teams, Word, Excel and Outlook.

Qualifications

• Overnight on-call capacity (10:00 p.m. – 7:30 a.m. ET).
• Excellent verbal and written communication skills.
• Ability to balance multiple scheduling demands across regions.
• Experience in customer service or similar role.
• Comfort with digital scheduling tools and Microsoft 365 apps.

Responsibilities

• Provide after-hours on-call support using company devices.
• Respond to scheduling-related calls and changes, including staff book-offs and urgent gaps.
• Escalate clinical concerns to the on-call nurse for active visits.
• Document all calls and actions in electronic records for billing and reporting.
• Maintain professionalism and sound judgment in time-sensitive situations.
• Manage inbound care service requests via phone and email.
• Schedule staff to ensure consistent family experiences.
• Participate in training and continuous improvement activities.
