Job summary

University Health Network (UHN) is Canada’s leading hospital system, with Toronto Western Hospital a key site. This permanent full-time outpatient role supports medication access and reimbursement for patients visiting UHN's Outpatient Pharmacy and Shoppers Drug Mart.

The successful candidate coordinates access processes, acts as a liaison among physicians, nurses and pharmacists, and provides guidance on programs and eligibility. Join a large team delivering patient-centered care within UHN.

Qualifications

• Completion of a two-year community college program in an Allied Health discipline.
• Minimum of two years practical experience in an outpatient setting.
• Knowledge of public and private medication access/reimbursement programs.

Responsibilities

• Serve as the centralized contact for physicians, nurses, social workers and pharmacists involved with medication access.

Salary Range: $65,910 - $82,387 per annum
Hours: 37.5 Hours per Week

Duties

• Facilitate and coordinate the submission of application forms for access to medication assistance for outpatients
• Serve as the centralized point of contact for physicians, nurses, social workers and pharmacists, and, will be actively involved with government, community based and private agencies that provide financial assistance for medications
• Serve as an advisor to patients, families and health care providers regarding medication access and reimbursement

• At minimum, completion of a two year community college program in an Allied Health related discipline or recognized equivalent required
• At minimum, two years practical and related experience
• Minimum of 2 years’ experience in rheumatology, oncology, ophthalmology or related Outpatient setting
• Knowledge and experience with public and private medication access/reimbursement programs and relevant agencies
• Knowledge in specialty drugs (eg. rheumatology, ophthalmology, hepatologyetc.); as asset
