Job summary

West Coast Seniors Housing Management is seeking a licensed practical nurse (LPN) to lead care delivery in a community under HEABC wage levelling. The LPN reports to the Director of Care and coordinates resident-centered care with physicians, families, and care teams.

This full-time permanent role offers a $10,000 signing bonus and relocation assistance up to $3,300, with preference for long-term care experience. Ongoing coaching and a collaborative team environment are emphasized.

Qualifications

• Graduate of an approved Practical Nursing program with active LPN license (BCCNM).
• Valid Occupational First Aid Level 1 certification.
• Successful annual RAI competency testing.
• Long-term care experience is an asset.

Responsibilities

• Coordinate care delivery and act as role model and team leader.
• Assess residents' needs and implement individualized care plans.
• Liaise with physicians, families, and staff to ensure quality resident-centered care.
• Administer prescribed medications and treatments within BCCNM limits.
• Document health changes and notify families and physicians as needed.
• Participate in interdisciplinary care conferences and care plan reviews.
• Support admissions, discharges, and confidentiality in daily operations.
• Provide staff orientation and on-the-job coaching to improve care.
