Job summary

Cogir Senior Living seeks a Licensed Practical Nurse to deliver person-centered nursing care for residents across our retirement residences. You will work within a multidisciplinary team to implement care plans, monitor residents’ conditions, and document assessments and outcomes.

You’ll mentor unregulated care providers, communicate effectively with families, and uphold provincial care standards while supporting a compassionate, resident-focused culture.

Qualifications

• Current registration as a Licensed Practical Nurse in the applicable province.
• Demonstrated sound clinical knowledge for geriatric care.
• Experience with seniors and with unregulated care providers preferred.
• Strong communication, empathy and teamwork in a multidisciplinary setting.
• Good organizational, time-management and documentation skills.

Responsibilities

• Provide planned, patient-oriented, goal-directed nursing care per provincial standards.
• Collaborate with the multidisciplinary team to support residents and families.
• Implement and evaluate care plans; adjust as required.
• Complete and ensure residents' assessments, care plans and documentation are accurate.
• Coach and mentor unregulated care providers.

How YOU Make a Difference

LPNs are responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.

LPNs are an integral part of our multi-disciplinary team who build trusted relationships with our residents, families and teams with continuous ongoing and effective communication.

LPNs implement and evaluate the effectiveness of the care plans and make changes as required.

LPNs complete and ensure resident’s clinical care assessments, care plans and documentation are accurate, up to date and communicate changes as required.

LPNs teach, delegate and direct unregulated care providers, providing them with coaching & mentorship.

What YOU Bring

• Current registration as a Licensed Practical Nurse in the applicable province
• Demonstrated sound clinical knowledge
• Experience working with seniors and with unregulated care providers preferred
• Education and experience in gerontology, rehabilitation, community/complex/long term care an asset
• Strong drive, respect and an understanding of the emotional and physical demands of the role
• Focus on teamwork and customer service with the ability to build trusted relationships
• Strong written and oral communication skills and critical thinking skills
• Demonstrated organizational and time management skills
• Computer proficiency and experience with Microsoft office applications
• Current Basic Cardiac Life Support and First Aid certification required
• Current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
