Job summary

Cogir Senior Living seeks a dedicated Registered Practical Nurse to deliver patient-centered nursing care for residents. You will work within a multidisciplinary team, coordinating with families and staff to ensure continuity of care.

The role requires updating care plans, maintaining accurate documentation and mentoring support staff. The successful candidate will have current RPN licensure, strong clinical knowledge in geriatric care, and the ability to handle the emotional and physical

Qualifications

• Must be a registered Practical Nurse with current provincial license.
• Experience caring for seniors and coordinating with unregulated care providers is preferred.
• Strong communication and teamwork skills are essential.

Responsibilities

• Provide planned, patient-oriented nursing care per provincial standards.
• Be an integral part of a multidisciplinary team and communicate with residents, families, and staff.
• Implement and adjust care plans as needed based on assessments and changes.
• Complete and update residents' clinical assessments, care plans and documentation.
• Educate, delegate to, and mentor unregulated care staff.

How YOU make a difference

• RPNs are responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.
• RPNs are an integral part of our multi-disciplinary team who build trusted relationships with our residents, families and teams with continuous ongoing and effective communication.
• RPNs implement and evaluate the effectiveness of the care plans and make changes as required.
• RPNs complete and ensure resident’s clinical care assessments, care plans and documentation are accurate, up to date and communicate changes as required.
• RPNs teach, delegate and direct unregulated care providers, providing them with coaching & mentorship.

What YOU bring

• Current registration as a Registered Practical Nurse in the applicable province
• Demonstrated sound clinical knowledge
• Experience working with seniors and with unregulated care providers preferred
• Education and experience in gerontology, rehabilitation, community/complex/long term care an asset
• Strong drive, respect and an understanding of the emotional and physical demands of the role
• Focus on teamwork and customer service with the ability to build trusted relationships
• Strong written and oral communication skills and critical thinking skills
• Demonstrated organizational and time management skills
• Computer proficiency and experience with Microsoft office applications

• Current Basic Cardiac Life Support and First Aid certification required
• Current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
