Job summary

AgeCare is seeking an experienced Licensed Practical Nurse (LPN) to join our team in Chestermere, AB. Reporting to the Director of Care, you will provide patient-oriented, goal-directed nursing care in line with provincial standards.

You will complete resident assessments, care/service plans, and documentation, while implementing and evaluating care plans. You will also assist with care conferences and coordinate moves in/out with the Director of Care, ensuring high-quality care and compliance.

Qualifications

• Practical Nurse Diploma and current Alberta LPN license.
• 3+ years of experience in seniors’ health.
• Current CPR training.
• Computer proficiency with Microsoft Office.

Responsibilities

• Work as part of an inter-professional team applying person-centered care.
• Complete and ensure resident assessments, care plans, and documentation are accurate and up to date.
• Implement and evaluate care plans and adjust as required.
• Assist with scheduling of resident care conferences and participate where applicable.
• Coordinate moves in/out and transfers with the Director of Care.
• Provide direct resident care and medication administration in compliance with policies.
