AgeCare is seeking an experienced Licensed Practical Nurse (LPN) to join our team in Alberta. Reporting to the Director of Care, the LPN will provide planned, patient-centered nursing care in accordance with provincial standards.

You will collaborate with an inter-professional team, complete assessments and care plans, and document changes. You will assist with transfers and ensure safe medication administration while upholding AgeCare's values of trust, respect, quality and teamwork.

Qualifications

• LPN diploma and current Alberta registration.
• Minimum 3+ years of experience with seniors’ health.
• CPR certification required.
• Proficient with Microsoft Office applications.

Responsibilities

• Work as a member of the inter-professional team applying the person-centred approach to care.
• Complete and ensure residents' clinical care assessments, care plans and documentation accurate and up-to-date.
• Implement and evaluate the effectiveness of care plans and make changes as required.
• Assist with scheduling of residents' care conferences and participate in conferences.
• Collaborate with the Director of Care to facilitate moves, transfers and admissions.
• Provide direct resident care and medication/treatment administration and document care per regulatory standards.

The Opportunity AgeCare is seeking an experienced and dynamic Licensed Practical Nurse (LPN) to join our team. Reporting to the Director of Care, this role presents a remarkable opportunity for YOU to make a significant impact on the lives of our residents and their families. The LPN is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.

What You Will Do

• Work as a member of the inter-professional team as described by our Professional Practice Model applying the person-centered approach to care.
• Complete and ensure resident's clinical care assessments, care/service plans and documentation are accurate, up to date and changes communicated as required.
• Implement and evaluate the effectiveness of the care/service plans and make changes as required.
• Assist with scheduling of resident's care conferences and participate in conferences where applicable.
• Work closely with the Director of Care to facilitate resident's move in, move out and transfers.
• Provide direct resident care and medication/ treatment administration and document care in compliance with regulatory and Resident Care standards and policies and procedures.

What You Bring

• Practical Nurse Diploma.
• Current registration and membership in good standing in applicable Province.
• 3+ years of experience with seniors’ health.
• Current CPR training.
• Computer proficiency and experience with Microsoft office applications.
• Demonstrates leadership, coaching and mentorship skills.
