Qualifications

• Completion of an accredited practical nurse education program.
• Active or eligible for registration with CLHA and a practice permit.
• Current Basic Cardiac Life Support - Health Care Provider certification.
• Minimum of two years nursing experience in Acute Care or Home Care (last five years).
• Willingness to work to full scope of practice and engage in continuous learning.
• Strong critical thinking and assessment skills; able to plan and evaluate care.
• Proficient with computer use, clinical devices and electronic documentation.
• Valid driver's license and access to a vehicle for work.
• Must complete nonviolent crisis intervention training.

Your Opportunity:

If you enjoy providing services to clients in a community-based setting, this position will be of interest to you. The Licensed Practical Nurse (LPN) in Home Care works as a member of an interprofessional health care team, utilizing the nursing process to provide direct nursing care to individuals across the lifespan. Working as a Case Manager, the LPN is accountable for assigned caseloads, with the primary responsibility of assessing client needs, determining service requirements, negotiating service options, care planning, and coordinating transitions across care settings. The LPN also performs interventions and restricted activities for clients as required according to client needs. This position may include palliative clients in caseload. There is also an expectation to be a mobile worker. This role requires travel of up to 100 km per day to care for clients in their homes or rural communities. You will be responsible for operating a laptop computer and mobile phone and for performing manual handling tasks (lifting, lowering, carrying, pushing, and pulling) as a regular part of this position. For example, lifting approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common in this role. This position is based out of Didsbury Home Care and serves the communities of Didsbury, Carstairs, Crossfield, the Cremona area, and surrounding rural communities.

Required Qualifications:

• Completion of an accredited practical nurse education program.
• Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA).
• Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

• Minimum of two years of current nursing experience in an Acute Care or Home Care setting within the last five years.
• Recent experience working with complex wounds, surgical drains, and IV (HPTP) therapy.
• Knowledge, skills, ability, and willingness to work to full scope of practice.
• Commitment to continuous learning relevant to LPN practice.
• Ability to utilize critical thinking skills when conducting initial and ongoing nursing assessments, care planning, and service delivery.
• Ability to use critical inquiry to assess, plan, and evaluate the effectiveness of interventions on client outcomes.
• Excellent verbal and written communication skills.
• Basic technology skills and competency in computer use, clinical electronic devices, and electronic documentation.
• Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification from the Heart and Stroke Foundation obtained within the past year.
• Valid driver's license and access to a vehicle in good repair for work-related purposes.
• Must be able to participate and complete all aspects of the nonviolent crisis intervention training (including physical demonstrations).

Preferred Qualifications:

• Recent work experience in a community-based setting; Home Care experience preferred.
• Experience working with clients with complex chronic diseases and palliative care needs.
• Experience in case management practice and completion of the McMaster AHS Case Management Education Program.
• Advanced Wound Care Certificate.
