Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Licensed Practical Nurse to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life. The Licensed Practical Nurse (LPN) provides safe and effective nursing care according to ALA policies and nursing care standards. Working within the full scope of practice, the LPN observes, monitors, reports, and documents changes in the client's physical, mental, and emotional state, assists the client with activities of daily living and provides the client and family with relevant information and teaching. The LPN is responsible to collaborate as a member of the health care team, delivering consistent quality care, to meet the needs of the client. This position is integral to the Short-Term Intervention Team (STIT), which provides clinic support, as well as community visits that require specialized skills IV therapy and wound VAC Therapy.

You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. You will play a key role in providing safe, high quality, patient and family centred care while reflecting the shared vision and values of the organization.

Required Qualifications

  • You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet.
  • The successful candidate will work independently in clients’ homes, which could include congregate settings, single family dwellings, condos and/or walk‑up apartments.
  • Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs.
Preferred Qualifications
  • Experience as an LPN.
  • Experience in Continuing Care.
  • Palliative care experience.
