We are looking for a Licensed Practical Nurse to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life. The Licensed Practical Nurse (LPN) provides safe and effective nursing care according to ALA policies and nursing care standards. Working within the full scope of practice, the LPN observes, monitors, reports, and documents changes in the client's physical, mental, and emotional state, assists the client with activities of daily living and provides the client and family with relevant information and teaching. The LPN is responsible to collaborate as a member of the health care team, delivering consistent quality care, to meet the needs of the client. This position is integral to the Short-Term Intervention Team (STIT), which provides clinic support, as well as community visits that require specialized skills IV therapy and wound VAC Therapy.
