Job summary

Qualifications

  • Licensed professional in Nursing, Social Work, Occupational Therapy, Physiotherapy or Nutritional Studies with five years of related experience.
  • Current registration or eligibility with the applicable professional association is required.
  • Valid driver’s license and reliable transportation are required; accommodations considered.

Responsibilities

  • Develop, implement, and evaluate comprehensive support plans tailored to individuals.
  • Facilitate supportive decision-making practices.
  • Collaborate with multidisciplinary teams including healthcare professionals and community partners.
  • Advocate for person-directed planning and rights of people with disabilities.
