Job summary

The Government of Nova Scotia invites applications for Intensive Planning and Support Coordinators (IPSCs). This role focuses on empowering individuals with disabilities through person-centred planning, trauma-informed practice, and collaboration with healthcare and community partners.

Work locations are Barrington Passage, Yarmouth, or Digby, with travel across the region. We seek compassionate, adaptable professionals with RN/LPN, Social Work, OT, PT, or Nutrition backgrounds and at least five

Qualifications

  • Licensed professional in nursing, social work, OT, PT or nutritional studies with 5 years of related experience.
  • Must be registered or eligible for registration with the applicable professional association.
  • Ability to develop, implement, and evaluate comprehensive support plans tailored to individual needs.
  • Skills in facilitating and promoting supportive decision-making practices.
  • Understanding trauma and its impact with adaptive support approaches.
  • Excellent verbal and written communication skills for accessible information sharing.
  • Proven ability to build trusting relationships with individuals, families, and partners.
  • Strong advocacy, cultural competence, and knowledge of disability rights and resources.

Responsibilities

  • Develop, implement, and evaluate comprehensive support plans tailored to individual needs.
  • Coordinate transitions from institutions to community living.
  • Build strong relationships with individuals, families, and support networks.
  • Collaborate with multidisciplinary teams and service providers.
  • Advocate for person-directed planning and rights-based approaches.
