Job summary

University Health Network (UHN) is seeking a Service Coordinator for the Inpatient Stroke Services within the Brain and Spinal Cord Rehab Program at University Centre, Toronto Rehab. This temporary/full-time role focuses on ensuring efficient patient flow, admissions, discharges, and staff leadership across a multidisciplinary team.

The position requires a Master's degree in a health-related field, a current Certificate of Practice, and 4–6 years of relevant experience in admission/discharge

Qualifications

• Master's degree in health-related discipline required.
• Current valid Certificate of Practice from relevant Health related College (e.g., OT, PT, RN, SLP, SW, RT, RD) required.
• 4–6 years of related experience with admission and discharge planning and case management.

Responsibilities

• Leads and participates in quality improvements and program development initiatives.
• Plans, organizes, directs, coordinates, and evaluates patient admission and discharge activities.
• Mentors staff and promotes a supportive learning environment within the unit.
• Serves as a clinical resource to inter-professional teams on complex patient care needs.
• Establishes routine review of department capacity and resource allocation.
• Engages staff, physicians and stakeholders in problem solving and ongoing education.
• Acts as Manager’s delegate on clinical and practice issues when the Program Manager is unavailable.
