Job summary

University Health Network is hiring a Registered Nurse for West Park Healthcare Centre, 0.5 FTE, permanent part‑time. The role focuses on stroke/neuro rehabilitation care, education, and collaborative inter‑professional planning.

The ideal candidate holds a Diploma in Nursing with registered status in Ontario, with neuroscience experience preferred. A flexible, patient‑centered approach is essential in a busy, multidisciplinary environment.

Qualifications

• Diploma in Nursing from an approved program.

Responsibilities

• Plan, manage, evaluate and coordinate comprehensive professional nursing care for patients and families.
• Plan and implement care for patients with stroke/neuro and their families.
• Educate patients, families and facilities about stroke/neuro care.
• Participate in inter-professional care planning and rounds.
• Provide leadership and mentorship to students and new staff on the unit.
• Participate in hospital-wide initiatives and professional development.
• Ensure patient safety and comply with hospital policies.

Position Summary

The role of the Registered Nurse is the promotion of health and the assessment of, the provision of care for and the treatment of health conditions by supportive, preventative, therapeutic, palliative and rehabilitative means in order to attain or maintain optimal function.

Duties

• Plan, manage, evaluate and coordinate comprehensive professional nursing care for patients and families within the context of an inter-professional practice and primary nursing model
• Use theoretical and empirical knowledge and the input of team members to plan and implement care for patients with stroke/neuroand their families
• Education of patients, family, referring facilities and other community providers regarding care of patients with Stroke/Neuro
• Participate in inter-professional team care planning forums, inter-professional rounds and family conferences
• Provide leadership, supervision and mentorship to students and new staff on the unit related to Stroke/NeuroRehabilitation nursing
• Participate in professional, program and hospital-wide initiatives, including professional development of self
• Compliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients.

• Diploma in Nursing
• Baccalaureate degree in Nursing from an approved nursing program preferred
• Member in good standing with the College of Nurses of Ontario
• Neuroscience nursing experience preferred
• Ability to perform the essential clinical duties of the job
• Ability leading others, including the effective management of challenging interpersonal situations
• Effective communication, problem-solving and decision-making and reliable work habits

• Computer literacy in word processing and Electronic Patient Record
