Job summary

UHN in Toronto is seeking a Service Coordinator for the Inpatient Stroke Services program. The role plans, directs and coordinates patient admissions and discharges while providing clinical leadership to staff across the unit.

The position requires a Master’s degree in a health-related discipline and current registration in a Health-related college, with 4–6 years of related experience and strong leadership abilities. Temporary full-time, up to Oct 29, 2027, based in Toronto Rehab.

Qualifications

• Master's degree in a health-related discipline or equivalent is required.
• Current valid Certificate of Practice from relevant Health-related College (i.e., OT, PT, RN, SLP, SW, RT, RD etc.) is required.

Responsibilities

• Leads and participates in quality improvements and program development.
• Plans, organizes, directs, coordinates, and evaluates patient admission and discharge activities.
• Mentors staff and fosters a supportive learning environment for health professionals.
• Acts as clinical resource for inter-professional teams regarding complex health care needs.
• Maintains department capacity reviews and coordinates team allocation.
• Engages with patients, families and external partners to problem-solve cases and provide ongoing education.
• When Manager unavailable, acts as delegate on clinical and practice issues.

Position Summary

The Service Coordinator is a program specific leadership role supporting the Inpatient Stroke in various aspects of the unit/program function. In accordance with established standards, professional requirements and as an integral member of the multidisciplinary health care team(s) the SERVICE COORDINATOR is responsible for the overall patient flow of the program inclusive of planning, organizing, directing, coordinating and evaluating activities related to patient admission and discharge. In addition, this role provides administrative and clinical leadership to all staff in a supportive, empowering manner while facilitating the effective and efficient admission and discharge of patients.

Duties

• Leads and/or participates in developing and implementing organizational initiatives regarding quality improvements and program development
• Plans, organizes, directs, coordinates, and evaluates activities related to patient admission and discharge
• Ensures efficient and effective unit/program operation by providing mentorship and promoting a supportive learning environment for health professions, physicians, nurses, support and administrative staff
• Acts as a clinical resource for the acute and/or community inter-professional teams by providing information based on clinical knowledge and theory/practice related to clients with complex health care needs
• Establishes and maintains a systematic review and assessment of department capacity.
• Service Coordinators consult with and engage appropriate staff members, physicians and others and they independently make decisions for the patient’s admissions/discharges and team allocation
• Service Coordinators develop and maintain fundamental strong relationships with patients/families, external partners and other stakeholders by consulting, problem solving cases and providing ongoing education
• When a Program Manager is unavailable, the Service Coordinator of a program may act as Manager’s delegate on clinical and practice issues of the program

Qualifications

Education:

• At minimum, completion of a Master’s degree in Health-related discipline or recognized equivalent required
• Current valid Certificate of Practice from relevant Health related College (i.e., OT, PT, RN, SLP, SW, RT, RD etc.), required

Experience:

• At minimum, over 4 years up to and including 6 years practical and related experience and/or 2 years on-the-job training required
• Knowledge and experience in admission and discharge planning and case management requiredKnowledge and experience using utilization management principles and length of stay management strategies requiredDemonstrated leadership abilities and experience requiredExperience in project management preferredExperience in program development and implementation preferredExperience working with interdisciplinary teams is requiredSpecialized experience with the rehabilitation stroke population the role is requiredExcellent decision making, organizational and time management skills requiredKnowledge of standards of practice and applicable healthcare legislation required

  Professional Affiliations/Memberships:

  • Membership in relevant Ontario College of Health-related discipline required
  • Membership in relevant professional agencies and associations preferred
