Job summary

Maamwesying North Shore Community Health Services Inc is seeking a Case Manager for the HART Hub in Sault Ste. Marie, ON.

You will provide culturally grounded, trauma-informed case management to Aboriginal adults with mental health, addiction, or homelessness concerns, and help develop personalized wellness plans. You will work with clients, families, Elders, Knowledge Keepers, and partners to promote stabilization and long-term recovery while ensuring identity, culture, and community

Qualifications

• Bachelor of Social Work or related degree with 2 years related work experience or equivalent life experience.
• Experience with Indigenous populations, mental health and addictions, trauma-informed care, and case management practices.
• Knowledge of local Indigenous communities, culture, language, and traditional health approaches or willingness to learn.
• Strong critical thinking and problem-solving skills.
• Ability to build rapport and communicate clearly with clients.
