Job summary

VON is seeking a Case Coordinator to provide comprehensive assessments and develop client-centered care plans. The role supports self-management and collaborates with clients, caregivers, and providers within a community care framework.

The hybrid position requires community presence 2–3 days weekly, with potential travel to Kingston and Trenton areas; strong organizational and communication skills are essential.

Qualifications

• University degree or college diploma in health or social services.
• At least 3 years of experience in home/community care.
• Experience with validated assessment tools and community resources.

Responsibilities

• Interpret and complete standardized assessment tools (interRAI CHA, supports Intensity Scale, or other tools).
• Conduct client assessments using conversation-based methods.
• Develop a comprehensive care plan with client and caregiver involvement.
• Identify and prioritize environmental risks to health and safety.
• Facilitate access to services and navigate resources to support the plan.
• Monitor progress and adjust care plans as required.
• Discuss client care plan expenditures and advocate for resources.
