Job summary

WNHAC in Ontario is seeking a Primary Care Nurse Care Coordinator to join a client-centered, inter-professional team. The role includes nursing assessments, immunizations, wound care, and medication administration within the nursing scope of practice.

You will collaborate with Home and Community Care staff and Community Health Nurses to support care measures, maintain medical records, participate in quality improvement, and travel to community sites as required.

Qualifications

• Must hold RN in good standing with the College of Nurses of Ontario.
• Immunization administration, venipuncture, wound care, and advanced nursing assessment skills are assets.
• Willingness to travel and work in a community-based Indigenous organization.

Responsibilities

• Carry out nursing assessment, nursing diagnosis, and treatment within scope.
• Engage in expanded clinical practice through training and clinical directives; includes screenings.
• Collaborate with the inter-professional team for wholistic client care and referrals.
• Support care coordination and timely review of diagnostics and age-related screenings.
• Maintain accurate medical records and documentation for accountability.
• Participate in quality improvement, risk management, and program development.
