Job summary

Circle of Care is seeking a Supervisor, Client Services to lead ongoing client assessments and care planning. The role includes supervising Home Service Workers, arranging family and team meetings, and ensuring timely communication of client status.

The position is full-time and on a 18-month contract, with hybrid work from the Toronto office and remote days. Qualified candidates are Regulated healthcare professionals with current registration and experience with elderly populations.

Qualifications

• Regulated healthcare professional with current registration in the applicable College.
• Experience working with elderly population or in community-based care.
• Able to supervise a team of Home Service Workers.

Responsibilities

• Ongoing assessment/reassessment of clients and care planning.
• Supervise Home Service Workers and conduct performance appraisals.
• Coordinate family and team meetings and on-call schedule.
• Ensure changes in client status are communicated promptly.
