Job description

JOB DEFINITION:

The mission of the CN Guidance FACT Team is to deliver comprehensive and effective services to individuals who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. The FACT Team provides an integrated set of evidenced-based treatment and support services through a highly individualized approach. Services are provided in the community in places and situations where problems arise.


EDUCATIONAL REQUIREMENTS:

Bachelor's Degree in social work, psychology, rehabilitation counseling or related field required.


EXPERIENCE REQUIRED:

One year experience working in a mental health setting.


DUTIES AND RESPONSIBILITIES:

  1. Assist FACT Team recipients in obtaining and maintaining adequate housing
  2. Must have an understanding of concrete services i.e. Medicaid, Medicare, DSS and SPOA housing.
  3. Completes referrals and linkage to community benefits, organizations and institutions that will assist the client in completing the stated treatment plan goals.
  4. Responsible for Quality Improvement, Utilization Review and Performance Improvement activities including but not limited to data collection, analysis of outcome trends, and identification of service provision, which needs improvement, completion of case records and the timely submission of required documentations.
  5. Plan and coordinate all services, in the office or in the field, which support and supplement the rendering of case management and care coordination services to clients including but not limited to applying for and maintenance of benefits through the Department of Social Services, Social Security Administration, housing applications etc.
  6. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress.
  7. Complete QI training during onboarding and participate in QI refresher training as needed.
  8. Actively engage in agency wide trainings and work groups as well as NYS specific trainings on and for the FACT model.
