Summary of Duties :

Reporting to the Manager(s) or designate, the Access Coordinator, Homeless Prevention provides clients at risk for homelessness with focused customer service which includes determining eligibility for social services and affordable housing supports; connects clients with community services and partners to promote ongoing housing stability and life stabilization.

  • Maintains knowledge of the Community Homeless Prevention Initiative, locally-driven housing projects and housing support programs and the Reaching Home guidelines, municipal policies and community resources.
  • Reviews and completes all necessary documentation in a Coordinated Access Intake and provides information to Managers to inform decisions.
  • Determines eligibility and makes recommendations to Manager when connecting individuals to vacancies in housing support programs, benefits and units.
  • Completes all necessary Province of Ontario and municipal documentation in compliance with regulations and guidelines.
  • Checks and assesses income, assets and circumstances to establish initial and continuing eligibility for housing support programs and other benefits administered through Coordinated Access.
  • Creates a housing stability and life stabilization plan and refers client to the appropriate agencies in order to assist in the shortest route to housing.
  • Utilises resources through the community and Coordinated Access to prevent and divert individuals and families from experiencing homelessness.
  • Responds to inquiries from clients, Community Partners, housing providers and housing support programs regarding housing issuesas assigned.
  • Meets with housing support programs, community partners, and clients to discuss issues and concerns pertaining to inquiries and escalated situations, as necessary.
  • Supports client in securing and retaining housing through accessing community supports outside the Coordinated Access System.
  • Completes comprehensive documentation regarding participant’s housing history and relevant circumstances for future consideration.
  • Assists in connecting individuals and families at risk of experiencing homelessness to shelter, outreach and other emergency supports.
  • Maintains all required documentation for data processing and statistical purposes.
  • Refers and advocates on behalf of clients to other agencies to access support resources.
  • Performs related duties as assigned.

Qualifications :

Completion of a two-year Social Service Worker diploma or equivalent.

Experience :

One year of previous related work experience is required. Previous work experience in housing services is an asset.

Specialized Training & Licenses :

Skills and abilities in the following areas are necessary :

  • Knowledge of housing support programs and related legislation, program and policy guidelines.
  • Demonstrated verbal and written communication skills and ability to work within a team work environment.
  • Demonstrated problem solving, decision making and basic numeric skills.
  • Computer literacy utilizing MS Office software applications, as well as utilizing word processing, spreadsheet and database applications.
  • Demonstrated ability to provide high quality customer service to clients in an equitable manner.
  • Valid Driver’s Licence - Class G

Compensation & Other Information :

49,450 - $69,530 (Level 9)

This posting is for 1 temporary full-time up to 1 year position.

Current hours of Work : Monday - Friday from 8 : 30 a.m. to 4 : 30 p.m.

Work Arrangement : Hybrid. Subject to change in accordance with business requirements.

These hours of work are subject to change in accordance with the Collective Agreement and may include evening hours and Saturdays.

NOTE : Applicants may be required to complete a job related test.
