Qualifications

• Membership in good standing with the applicable regulatory body (RN, Physiotherapist, Occupational Therapist, Social Worker, Speech-Language Pathologist).
• 2+ years of recent experience in community health or related field.
• Knowledge of the health care delivery system and community resources.
• Excellent interpersonal, communication, assessment, problem-solving and decision-making skills.

Responsibilities

• Assess care needs and eligibility; develop individual care and service plans with patients and families.
• Link patients with service providers and coordinate care plan delivery.
• Establish a supportive relationship with patients and families; respect values and preferences.
• Balance patient needs with available resources; ensure proper documentation and reporting.

Education
