Ontario Health atHome in the Algoma region is hiring a Care Coordinator (RN, PT, OT or SW) for a full‑time role based at Sault Ste. Marie and the Sault Area Hospital site.

You will assess patient care needs, determine eligibility for services, and arrange referrals to community resources. You will coordinate and monitor care plans, link patients with service providers, and support patients and families in a collaborative, multidisciplinary team.

Qualifications

  • Membership in good standing with applicable regulatory bodies (nursing, PT, OT, SW).
  • Experience in community health or related field.
  • Excellent interpersonal, communication, assessment, problem-solving and decision-making skills.
  • Effective time management and organizational skills; able to work independently in a multidisciplinary setting.

Responsibilities

  • Assess care needs, determine eligibility for services, and develop individual care plans in collaboration with patients and families.

As a Care Coordinator (Full-Time, Start Date: Immediately Location: Sault Ste. Marie – Located in Sault Area Hospital ), you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners .

What will you do?

What must you have?
  • Membership, in good standing, with the applicable regulatory body:
  • College of Nurses of Ontario
  • College of Physiotherapists of Ontario
  • College of Occupational Therapists of Ontario
  • Ontario College of Social Workers and Social Service Workers
  • Recent experience in community health or a related field
  • Knowledge of the health care delivery system and community resources
  • Excellent interpersonal, communication, assessment , problem-solving and decision-making skills
  • Effective time management , prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment
  • Established ability to accurately complete required documentation, reports and forms
  • A valid driver’s license and access to a reliable vehicle
  • Proficient in a Windows environment
  • Advance oral and written proficiency in English is essential
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.
What would give you the edge?
  • Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language

Salary: $41.29 to $51.13 as per the ONA Collective Agreem
