Job summary

Ontario Health atHome seeks a Care Coordinator (RN, PT, OT or SW) in Timmins to assess patient needs, determine eligibility, coordinate services, and engage patients and caregivers. Work across home, office or facility settings with a multidisciplinary team.

Requirements include active licensure with a regulatory body, community health experience, and strong communication and documentation skills. Fluency in English and French is preferred; a valid driver’s license and vehicle access are

Qualifications

  • Must be a member in good standing with the applicable regulatory body (e.g., CNO, CPSO, COTO, OCSWSSW).
  • Recent experience in community health or related field.
  • Knowledge of health care delivery system and community resources.
  • Excellent interpersonal, communication, assessment, problem-solving and decision-making skills.
  • Effective time management, prioritization and organizational skills; able to work independently and with a team.
  • Accurate documentation, reports and forms handling.

Responsibilities

  • Assess care needs with patients and families, determine eligibility for services, and develop individual care plans.
  • Link patients with service providers and coordinate care plan delivery.
  • Establish relationships with patients and families; balance needs with available resources.
  • Initial assignment to Hospital with possible changes based on needs.

As a Care Coordinator (Full-Time, Permanent; Start date: Immediately; Location: Timmins and DistrictHospital), you will assess and determine patient care needs and eligibility, provide access and referralsto community services, and engage with patients, caregivers and other health care practitioners.

Whether you work in our office, in the community, or a health care facility – you will play a lead role inproviding connected, accessible, patient-centered care – and be supported by our collaborative teamthat includes over 9,000 regulated health care and other professionals.

What will you do?
  • In collaboration with patients and their families, assess care needs, determine eligibility forservices, and develop individual care and service plans
  • Link patients with service providers
  • Coordinate and monitor care plan delivery
  • Establish a helping relationship with patients and their families
  • Balance patient needs and choices with available resources, ensuring patients’ values andpreferences are respected
  • Initial assignment Hospital, with possible assignment changes within the occupation based onoperational needs
What must you have?
  • Membership, in good standing, with the applicable regulatory body:
    • College of Nurses of Ontario
    • College of Physiotherapists of Ontario
    • College of Occupational Therapists of Ontario
    • Ontario College of Social Workers and Social Service Workers
  • Recent experience in community health or a related field
  • Knowledge of the health care delivery system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving and decision-makingskills
  • Effective time management, prioritization and organizational skills, with the ability to workindependently and co-operatively in a busy multidisciplinary environment
  • Established ability to accurately complete required documentation, reports and forms
  • A valid driver’s license and access to a reliable vehicle
  • Proficient in a Windows environment
  • Advance oral and written proficiency in English and French is essential
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, allemployees are required to submit proof of COVID-19 vaccination status prior to start date.

  • Experience working with diverse patient groups, e.g., multicultural, homeless, palliative,acquired brain injury (ABI), mental health, geriatrics, pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language
