Job summary

Ontario Health atHome in Ottawa is seeking a bilingual Hospital Care Coordinator to assess patient needs, determine eligibility, arrange service planning, and coordinate with families and health care partners. You will work across office, community, or hospital settings to support safe hospital-to-home transfers.

You will be responsible for developing care plans, coordinating resources, and ensuring timely discharge planning, with a 35-hour work week and a starting salary of CAD 44,480–47,517.

Qualifications

• Registered professionals with CSA Ontario college registrations or equivalents
• Minimum 2 years related professional experience
• COVID-19 vaccination status required before start date

Responsibilities

• Assess referred clients for admission and establish a helping relationship
• Perform multi-dimensional assessment and identify need for crisis intervention
• Develop and adjust care plan in collaboration with client and care team
• Coordinate access to resources and link with service providers
• Initiate and authorize implementation of the care plan
• Monitor and reassess care plans for quality and outcomes
• Conduct discharge planning and redirect to alternative resources
