Job summary

Ontario Health atHome is seeking a Bilingual Hospital Care Coordinator to assess patient needs, determine eligibility, and arrange access to home care services. The role involves collaborating with patients, families, and hospital partners to develop a safe, effective care plan.

You may work in office, community, or a health care facility, with a focus on patient-centered care and seamless hospital-to-home transfers.

Qualifications

• Registered Nurse with BScN or a nursing diploma and registered with the appropriate Ontario college.
• Minimum of 2 years related professional experience.
• COVID-19 vaccination status as required by policy.

Responsibilities

• Assess referred clients for admission and determine eligibility.
• Develop a care plan in collaboration with clients and health professionals.
• Coordinate access to community resources and referrals.
• Monitor, reassess, and discharge planning of care.
• Collaborate with hospital partners to support safe hospital-to-home transfer.
