Job summary

VON Canada is seeking a Visiting Nurse to deliver holistic, client-centred care in home and community settings across ages, from children to seniors. You will assess needs, develop care plans, collaborate with other healthcare professionals, and apply wound care, IV therapy, and palliative support as appropriate.

This role requires RN licensure in the province, a valid driver's license with vehicle and insurance, PPE compliance, and the ability to work days, evenings, weekends, and on-call.

Qualifications

• Must be licensed as a Registered Nurse in the province of practice.
• Community health experience is an asset.
• Must enjoy driving, have a personal vehicle and possess both a current driver's license and proof of vehicle insurance.
• Must be able to wear Personal Protective Equipment (PPE).
• A satisfactory Criminal Records Check is required; a Vulnerable Sector Search and/or Child Abuse Registry Check may also be necessary.

Responsibilities

• Client assessments to develop individual care plans.
• Provide primary care in collaboration with other healthcare professionals.
• Wound care, including simple and advanced techniques.
• Chronic Disease Management and education (e.g., diabetes care).
• IV therapy, CVAD management, phlebotomy and specimen collection.
• Dialysis management and rehabilitative programs.
• Palliative and End of Life Care and medication management.
