Job summary

VON Canada is seeking a Visiting Nurse to deliver holistic, person-centered care in clients' homes, clinics, or virtually, with autonomy and support from a multidisciplinary team. Qualifications

• Must be licensed as a Registered Nurse in the province of practice.
• Community health experience is an asset.
• Must enjoy driving and have a personal vehicle with a valid licence and insurance.

Responsibilities

• Conduct client assessments to develop individual care plans.
• Provide primary care in collaboration with other healthcare professionals.
• Administer wound care, IV therapy, and manage chronic disease care.
• Support palliative and end-of-life care, along with medication management.
