Job summary

The Care Company Inc. is seeking Visiting Nurses (RN/RPN) to join our Home & Community Care team. Travel within Central Ontario areas with casual, flexible scheduling and a broad scope of community nursing services.

As part of the Visiting Nursing Team, you will provide skilled nursing interventions, wound care, medication administration, education, and collaboration with an interdisciplinary team to support clients at home.

Qualifications

• Active registration and good standing with the College of Nurses of Ontario (CNO) as an RN or RPN.
• Current CPR and First Aid certification.
• Current WHMIS certification.
• Negative TB test meeting onboarding requirements.
• Criminal Record Check with Vulnerable Sector Screening.
• Ability to communicate effectively in English, both verbally and in writing.

Responsibilities

• Complete comprehensive nursing assessments and develop, implement, evaluate, and update plans of care.
• Manage a community caseload and act as point-of-care lead when designated.
• Provide skilled nursing interventions in clients' homes, including wound/ostomy care and medications.
• Administer medications per orders and evidence-based practice.
• Educate clients and caregivers on medications, disease management and safety at home.
• Maintain accurate electronic documentation and ensure infection prevention and control.
• Coordinate with hospitals, physicians and other professionals during care transitions.
