Job summary

Access Healthcare Services Inc. is seeking a Registered Nurse or Registered Practical Nurse for in-home visits in Cornwall, Ontario. You will provide client-centered care, complete assessments, and develop holistic care plans in collaboration with a multidisciplinary team.

The role supports full scope practice and adherence to care standards, with flexible hours and on-call rotation. Applicants must be in good standing with CNO, vaccinated for Covid-19, have a vehicle and CPR, and pass TB and

Qualifications

• Registered Nurse or Registered Practical Nurse in good standing with CNO.
• Proof of Covid-19 vaccination.
• Vehicle required with valid driver’s license.
• Car insurance with a liability of $2 million.
• Valid CPR.
• Step 2 TB Test (Completed within the last 2 years).
• Vulnerable Sector Police Check (less than 3 months old).
• Excellent assessment, documentation, verbal, and communication skills.

Responsibilities

• Provide in-home, high quality client centered care.
• Complete comprehensive client assessments to develop collaborative, holistic care planning.
• Work to the full scope of practice as outlined by the CNO Professional Standards and AHS policies and procedures.
• Upholds the mission, vision and values of AHS.
• Adheres to infection, prevention & control measures and nursing best practice standards.
• Participates in ongoing continuing education and professional development.
• Reports, consults and collaborates with members of the interdisciplinary health care team.
• Participates in the on-call nursing rotation.
