Job summary

The Care Company is seeking compassionate Registered Nurses (RN) and Registered Practical Nurses (RPN) to join our Visiting Nursing Team in Ontario. This mobile role involves traveling between client homes to deliver skilled nursing care, post-hospital support, wound care, and medication management within a defined geographic area.

You will maintain detailed clinical records and collaborate with an interdisciplinary team to ensure high-quality, person-centred care.

Qualifications

• Active registration and good standing with CNO as RN or RPN.
• CPR and First Aid certification current.
• WHMIS certification current.
• Negative TB test per onboarding.
• Criminal Record Check with Vulnerable Sector Screening.
• Ability to communicate effectively in English, verbal and written.
• Valid driver’s license and vehicle for client-home travel.
• Ability to work independently and also with a care team.
• Strong clinical assessment, critical thinking, and decision-making skills.

Responsibilities

• Perform nursing assessments and develop care plans.
• Manage a community caseload and collaborate with the healthcare team.
• Provide in-home skilled nursing interventions and wound/catheter care.
• Administer medications per orders and monitor vitals.
• Educate clients and caregivers on medications and disease management.
• Coordinate care transitions with hospitals, physicians, and Ontario Health atHome.
• Maintain accurate electronic documentation and ensure infection control.
• Organize visits and communicate scheduling needs with the Care Company.
