Job summary

Carefor Health & Community Services in Ottawa is seeking a Registered Practical Nurse to provide high-quality home care and community support to clients in their homes and local settings.

You will implement care plans, assess clients and families, and teach care techniques. Collaboration with the health care team and ongoing professional development are essential to promote client-directed care and independence.

Qualifications

• Registration as a Registered Practical Nurse in the Ontario College of Nurses.
• For positions requiring expertise in a specific field of clinical practice additional degrees/certificates may be preferred or required.
• Clinical practice experience preferred.
• Experience in community health preferred.

Responsibilities

• 45 hours Bi-Weekly
• Utilizes interview and communication skills to obtain information from client/family
• Provides complete assessment of client/family and environment
• Documents assessment findings according to established procedures
• Contributes to and updates the client’s care plan with the health care team
• Documents care plan modifications and rationales (e.g. visit schedule)
• Implements strategies from care plans in consultation with client/caregiver and health team
• Documents client/family responses to care
• Describes and evaluates responses to care
• Performs health teaching to clients and families using adult learning principles
• Integrates current research into clinical practice
• Documents clearly and according to Carefor standards
• Operates within Standards of Nursing Practice (Ontario College of Nurses)
