Job summary

VHA Home HealthCare (VHA) is seeking an Occupational Therapist to assess and support adult clients in their homes across Ontario. You will coordinate care with the client care team, plan interventions, and promote independence using evidence‑based practices.

Responsibilities include documenting progress, maintaining professional standards, and delivering compassionate, client‑centred care while leveraging virtual care when appropriate and managing a diverse caseload.

Qualifications

• Degree in Occupational Therapy from a recognized school.
• Member in good standing with the College of Occupational Therapists of Ontario.
• Valid Driver’s License with access to a vehicle.
• Ability to work independently with caseload management.
• Experience with electronic documentation and virtual care.

Responsibilities

• Assess and implement occupational therapy interventions per guidelines.
• Implement client interventions based on assessment and care plans.
• Ensure evidence-based services and outcome measurement.
• Coordinate care with client’s team and schedule interventions.
• Document assessment findings, treatment plans and progress.
