Job summary

VHA Home HealthCare, a leading not-for-profit home care provider in Ontario, seeks an Occupational Therapist for adults to assess and support clients in homes and schools. You will deliver evidence-based interventions, coordinate with care teams, and promote independence for pediatric and adult clients.

Qualifications

• Degree in Occupational Therapy from a recognized school.
• Member in good standing with the College of Occupational Therapists of Ontario.
• Valid Driver's License with access to a vehicle.
• Compassionate and empathetic
• Self-driven with ability to work independently
• Skilled in caseload management
• Skilled at electronic documentation
• Able to provide virtual care
• An ADP authorizer (asset)

Responsibilities

• Assess and implement occupational therapy interventions in accordance with professional practice guidelines and standards established by VHA and regulatory college.
• Implement client interventions based on assessment information and client input in conjunction with the client's integrated care plan.
• Ensure services provided are best practice and evidence-based with regular outcome measurements.
• Coordinate care with the client care team regarding plans and schedules for interventions.
• Document all assessment findings, treatment plans, progress and discharge summaries as required.
• Support safe client care through knowledge and continuous quality improvement.
