Job summary

The Rekai Centres in Toronto seeks an experienced RN-EC (Nurse Practitioner) for two full-time positions at our downtown LTC homes. You will lead comprehensive resident health assessments, develop individualized care plans, and provide direct clinical care within the NP scope, in collaboration with physicians and the care team.

Ideal candidates hold CNO RN-EC registration, CPR/First Aid, gerontology knowledge, and a strong commitment to elder care in an on-site, non-remote setting.

Qualifications

• Current registration with the College of Nurses of Ontario as an RN-EC for Primary Health Care or Adult and eligible to practice without restrictions.
• CPR and Standard First Aid certification for Health Care Professionals.
• Advanced knowledge in assessment, diagnosis, and care management of older persons with delirium, depression, cognitive impairment.
• Vulnerable Sector Check valid within the last 6 months.

Responsibilities

• Conduct comprehensive health assessments, including physical exams and med history reviews.
• Develop and implement individualized care plans based on residents’ needs and goals.
• Provide direct clinical care within Nurse Practitioner scope, including tests, prescriptions, and minor procedures.
• Collaborate with physicians, RNs, allied health professionals, and other teams to optimize outcomes.
• Educate residents and families on health promotion and self-management strategies.
• Participate in meetings and quality improvement initiatives to enhance resident care and safety.
• Maintain accurate documentation in eMAR and adhere to regulatory standards.
