Job summary

RhynoCare is seeking an experienced Nurse Practitioner to provide primary health care to residents in Long Term Care facilities in Etobicoke, Ontario. You will collaborate with a multidisciplinary team to ensure well-being and comfort for all patients.

The role emphasizes 80% direct resident care with on-call duties, leadership in care plans, and close interaction with families and discharge planners within CNO guidelines.

Qualifications

• Registration with the College of Nurses of Ontario (CNO) in the Extended Class.
• Bachelor of Science in Nursing (BScN); MSN with NP specialization preferred.
• Membership with Nurse Practitioners' Association of Ontario (NPAO) preferred.
• Professional liability insurance (RNAO or other).
• Vulnerable Sector Check current.
• CPR certification.
• Knowledge of RNAO Best Practice guidelines and MLTC regulations.

Responsibilities

• Delivers primary health care to residents in LTC.
• Provide on-call coverage for duties and schedules as required.
• Lead LTC team with admission/discharge planning and end-of-life care.
• Conduct health assessments, physical exams, and diagnostic reasoning.
• Develop person-centered care plans with residents and families.
• Collaborates with discharge planning services for transitions to the LTC home.
• Participates in QA programs, education, and research initiatives.
• Allocate ~80% of time to direct resident care and ~20% to education/research.
• Apply diversity and cultural safety in assessment and management.
