Job summary

Yee Hong Centre for Geriatric Care in Toronto, Ontario, seeks a Nurse Practitioner (Long Term Care Homes) to promote wellness, deliver advanced nursing care, and lead resident care through autonomous practice and interdisciplinary collaboration.

The NP will work within Yee Hong's mission to provide compassionate, person-centered care across diverse communities, contributing to education, leadership, and research within the organization.

Qualifications

• Current Registration with the College of Nurses of Ontario (CNO) in the Extended Class (Primary Health Care or Adult) and entitled to practice
• Masters Degree in Nursing (MScN)
• Specialty certification in gerontological nursing is an asset; Post-graduate cert and experience in palliative and end-of-life care are assets
• Experience practicing as an RN (EC) in a long-term care or geriatric setting is preferred
• GPA, PIECES or other seniors-focused training or professional development is an asset
• Strong computer skills including Point Click Care and MS Office

Responsibilities

• Direct resident care for clinical assessment and care planning
• Determine resident health care needs in consultation with physician
• Develop/initiate/modify resident care plans and therapies
• Initiate referrals for consultation, e-consult and virtual care
• Provide consultation to nursing, medicine and other team members
• Lead and participate in quality improvement and best practices within the LTC home
