AgeCare is recruiting a Nurse Practitioner to join Generations Toronto, a not-for-profit long-term care campus delivering resident-centered care in a 122-bed home. The role works collaboratively with physicians, the director of care, and interdisciplinary teams to meet individual resident needs.
You will assess, diagnose, and manage acute and chronic conditions, provide safe treatments, and lead clinical care planning, including mobility, palliative care, and family communication.

The campus includes a 122-bed Ismaili Ethnocultural Designated Long-Term Care Home. AgeCare has been appointed as the manager of the Generations Toronto long-term care home and is recruiting the Nurse Practitioner on behalf of Generations Toronto.
The Nurse Practitioner will work collaboratively on the delivery of resident care to meet individual needs within his/her level of competency. This role provides quality and safe resident care which helps to support and align resident needs.
Salary Range: $100,000.00 - $115,000.00 per year
Salary placement based on qualifications and experience.
