Job summary

The Psychogeriatric Resource Consultant (PRC) for the Nipissing/Temiskaming hub will partner to lead an integrated system of care through capacity enhancement, network engagement, and specialized clinical consultation. The PRC travels across LTCHs, community and acute care settings, applying BSO frameworks to support older adults with neurocognitive and mental health needs.

Reporting to the NE BSO Regional Manager, the PRC will champion knowledge-to-practice, contribute to quality improvement

Qualifications

• Minimum Bachelor’s degree in a recognized health care profession; Master’s degree preferred.
• Certifications in P.I.E.C.E.S., U-FIRST, GPA, Dementability, and BSO Foundations eligible to obtain.
• Experience with dementia, delirium, mental health, and neurocognitive conditions; knowledge-to-practice promotion.
• Demonstrated facilitation, coaching, and change management in diverse healthcare settings.
• Valid driver’s license and ability to travel within Nipissing/Temiskaming districts.

Responsibilities

• Lead integrated system of care for Nipissing/Temiskaming hub.
• Provide specialized clinical consultation and education across healthcare sectors.
• Promote knowledge-to-practice and support quality improvement initiatives.
• Travel throughout district and beyond as needed.
