Job summary

Providence Care Centre is seeking a part-time Personal Support Worker to join the interprofessional Seniors Mental Health Behavioural Support Services Mobile Response Team (MRT). You will provide direct, person-centred care to older adults in LTCH settings or during transitions, under RN/RPN supervision.

Must possess PSW certificate, be eligible to work in Ontario, and have LTCH experience. A valid G-class licence and vehicle access are required.

Qualifications

• PSW certificate from a recognized institution.
• Eligibility to work as a PSW in an Ontario long-term care home.
• Minimum two years of LTCH experience; equivalent experience considered.

Responsibilities

• Provide person-centred care and activities of daily living with MRT RN/RPN guidance.
• Support clients in the community or during LTCH transitions for seamless care.
• Deliver direct care including personal care, nutrition, mobility, and safety monitoring.
• Collaborate with LTCH teams and community partners to support behavioural care.
• Build therapeutic relationships with clients, families and care providers.
