Job summary

Extendicare is hiring an RPN to coordinate the Behavioral Supports Ontario program within our long-term care home. You will lead interdisciplinary rounds with social workers, pharmacists, physicians, and partners to drive quality care and safe behaviors.

You will complete required reporting, set improvement goals, and analyze data on quality indicators and medication use, contributing to a compassionate, resident‑centered environment across Canada.

Qualifications

• Registered as an RPN in good standing with the College of Nurses of Ontario.
• CPR and First Aid certifications are required.
• Extensive knowledge of PIECES, Gentle Persuasive Approach, and Montessori.
• 3–5 years experience in long-term care.
• Experience with dementia, delirium, mental health, addictions, and behavioral change scenarios.
• Commitment to the goals and principles of the BSO program.
• Canadian Gerontology certificate is considered an asset.

Responsibilities

• Coordinate the BSO interdisciplinary team meetings and rounds with social work, pharmacy, physicians, and community partners.
• Participate actively with the Professional Advisory Committee, Medical Advisory Committee, Quality Committee and other teams.
• Complete required home reports and documentation related to behaviors; ensure follow-up.
• Set goals, create action plans and monitor progress.
• Analyze and present detailed reports on quality indicators and medication use.
