Southbridge Care Homes in Ottawa, Ontario, is seeking a caring Behavioral Support Nurse – RPN to join our team as BSO – Restorative Care Lead. You will coordinate with the Director of Care to develop person-centered care plans that enhance safety, health, and happiness for residents.

The role requires collaboration with clinicians, administrators, and family stakeholders to support responsive behaviors. The position emphasizes hands-on resident care, assessments, and quality improvement

Qualifications

• Registered Practical Nurse with current certificate of competence from the College of Nurses of Ontario.
• Two+ years of experience in a similar role, preferably in long-term care.
• Bachelor’s/Degree in psychology or behavioral sciences would be an asset.

Responsibilities

• Contribute to delivering person-centered care and promote residents' abilities and self-esteem.
• Conduct assessments using tools to describe behaviors and identify risks and caregiver strategies.
• Ensure care plans reflect responsive behaviors and mitigate risks with weekly rounds.
• Communicate with external stakeholders and management to identify risks.
• Provide monthly reports and participate in quality improvement activities.
• Provide coaching to staff to ensure collaborative care for responsive behavior.
• Attend QA program meetings and participate in staff development.
• Collaborate with pharmacists and physicians on medication impacts on residents.
