Job summary

SOAHAC, Southwest Ontario Aboriginal Health Access Centre, London site, seeks a Registered Practical Nurse (RPN) for a full-time contract to cover parental leave through February 19, 2027. The role supports primary care teams with assessments, immunizations, venipuncture, wound care and EMR documentation.

Requirements include active CNO registration, an RPN diploma and 2–3 years of primary care experience. Travel to SOAHAC sites may be required; HOOPP pension option available.

Qualifications

• Current registration as a Registered Practical Nurse with the College of Nurses of Ontario (CNO)
• Graduate from a recognized RPN program
• 2–3 years clinical experience; preferably in a multi-disciplinary team setting
• 2–3 years previous experience working as an RPN in primary care, chronic disease management, preventative care, mental health and addictions

Responsibilities

• Assists with direct client care procedures and related tasks such as checks in clients, obtaining history, vital signs, updating and preparing charts
• Conducts intake appointments with new SOAHAC clients ensuring a holistic process
• Advocates on behalf of the client with regards to access to traditional healing and western health services as directed by client
• Performs specialized procedures, such as obtaining and processing lab specimens
• Maintains database/registry of client’s eligible/due for cancer screening, diabetes care, and immunizations in EMR
• Triage calls, provides telephone support, and arranges follow-ups with providers
